Standard Bankruptcy Forms Revised 5/12/2009

     Effective December 1, 2008, the Judicial Conference of the United States approved revisions to the following official Bankruptcy Forms:  (1) Official Form B1D, Individual Debtor's Statement of Compliance with Credit Counseling Requirement (Exhibit D);  (2) Official Form B8, Chapter 7 Individual Debtor's Statement of Intention; (4) Official Form B10, Proof of Claim; (3) Official Form B23, Debtor's Certification of Completion of Post-Petition Instructional Course Concerning Personal Financial Management; and (5) Director's Procedural Form B 201, Notice to Consumer Debtor(s) Under § 342(b) of the Bankruptcy Code.  The revised forms are available from the Eastern District of Michigan Bankruptcy Court's website at:  http://www.mieb.uscourts.gov/notices/2008amendforms.htm

     The Judicial Conference of the United States exists pursuant to statute, 28 U.S.C. Sec. 331.  The fundamental purpose of the Judicial Conference is to make policy with regard to the administration of the U.S. Courts. The Judicial Conference also supervises the Director of the Administrative Office of the U.S. Courts in the performance of his duties as the administrative officer of the courts of the United States under 28 U.S.C. § 604. In addition, certain statutes authorize the Judicial Conference to act in a variety of specific areas dealing with the administration of the courts.
